DCYF Waiver Request Form
The purpose of this form is to identify DCYF Core Academy courses that a new DCYF staff member may permanently waive due to past experiences or qualifications. The criteria that qualify for a permanent waiver are up to the discretion of the DCYF staff member’s supervisor, DCYF leadership, and CWEP. This form will not be used for temporarily waiving a course. This form is for internal use only and will not be shared with anyone outside of CWEP without the consent of the DCYF supervisor.
CWEP New Training/Curriculum Update Request Form
The purpose of this form is to propose a training for DCYF staff and/or DCYF partners to be delivered by CWEP. Please enter each field to the best of your ability. Once you submit the form, a representative from CWEP will be in touch with you for the next steps.
Bridges Credit Request Form
The purpose of this form is to request Bridges training credit for a training that you attended, or to submit a training attendance sheet for credit on behalf of a group.
**Please include your name and the current date in the file name of all documents that you submit for training credit or you will not receive credit**
