Authorization to Release Education Records

Community Standards must obtain a signed release form authorizing an official to release records or disclose information contained in education records to a non-affiliated third-party. 
RELEASE OF INFORMATION

A waiver of privacy rights awarded in the Family Educational Rights and Privacy Act (FERPA). Current and former students must complete and submit a signed release authorizing Community Standards to disclose information about a specific disciplinary record or contents of their entire disciplinary history with non-affiliated third parties (e.g., parents, guardians, advisors, potential employers, etc.).

Download, complete and submit the available release form. If the student is in need of a specific case number or guidance, contact the Office of Community Standards prior to submission.
