Benefits – Important Dependent Eligibility Verification Audit

USNH is conducting a dependent eligibility verification audit for employees whose dependents are covered under USNH medical and/or dental plans.
The audit is a cost savings measure to ensure that only eligible dependents are covered under the USNH medical and/or dental plans. For the audit, employees will be required to provide documentation of proof of eligibility for each dependent covered.
Failure to respond will result in dependents being dropped from benefits coverage.
Employees can mail or fax documents to:

Employees can also deliver them to Human Resources at 2 Leavitt Lane.
