USNH offers additional voluntary insurance plans for benefits eligible employees

Life Insurance & Accidental Death and Dismemberment (AD&D)  

Additional life and AD&D insurance may be purchased by employees for themselves and/or family members.  Contact the Office of Human Resources for application materials.

Short-Term Disability Insurance

Short-term disability insurance provides income protection for qualified absences due to non-work related injury or illness for up to 26 weeks (includes maternity leave). For more information and any plan criteria, please view the Plan brochure.

 Voluntary Long Term Care Insurance

Genworth Life Insurance Company (Genworth Life) is the provider for USNH employees enrolling in a Voluntary Group Long Term Care Insurance Program.

  • Current benefits eligible employees - the enrollment period for this new benefit is March 18, 2013 through April 12, 2013.  Current benefits eligible employees under age 66 and enrolling during this timeframe are automatically accepted without having to provide proof of good health.
    • Please NOTE:  Benefits eligible employees can enroll in this benefit at anytime throughout the year, however if you opt to enroll for this new benefit AFTER April 12, 2013, you will be required to provide proof of good health.
  • Newly hired benefits eligible employees (hired after March 18, 2013) will also have the opportunity to enroll (depending on age and the plan they choose) for coverage during their first 31 days of employment without having to provide proof of good health.
