Welcome Letter for Incoming Students from the Executive Director of Health Services

Dear UNH Student/Parent:
The staff of Health Services is pleased to welcome you to the University of New Hampshire. Please visit our website at http://www.unh.edu/health-services and click on Entrance Requirements: Required Health Forms and Information for Incoming Students. This will give you access to the forms that you need to read, complete and return to us. It is required that these forms be completed by July 1, 2008.
To summarize :
- A Physical Assessment Form must be completed by all first year students and mailed back to UNH Health Services by July 1st.
- Immunization Record:
- • It is the University's policy that all students are adequately immunized against MMR (measles, mumps and rubella) prior to attendance. Students born before 1957 are exempt from providing documentation and may note this on the online Health History Form under Immunizations.
- • Please complete the "Tuberculosis Risk Questions" located on the online Health History Form. If you answered "yes" to any questions, you need to discuss this with your health care provider and submit further information (per the paragraph at the bottom of that page).
- • Please discuss with your health care provider the risk of meningococcal disease. We support the American College Health Association's recommendation that all college students be vaccinated against meningitis to protect themselves against this serious disease.
- • We are also strongly recommending that all students be vaccinated against Hepatitis B.
- Incoming students are required to complete an online Health History Form which is available on our website.
- It is very important that you read the Patients' Bill of Rights and Responsibilities and Notice of Privacy Practices. UNH Health Services is committed to protecting the privacy of your health information. We provide you with a notice about our privacy practices to explain how, when, and why we use and disclose your health information. The on-line registration process directs you to read both of these documents.
- Beginning this year, UNH is requiring health insurance as a condition of enrollment for full-time students. For information, visit our website at http://www.unh.edu/vpsas/health.html.
Please note that for any services received here, you must show your UNH student identification (ID) card. Be sure to bring your ID card with you each time you visit.
